Use our Online Vote-by-Mail Request Form below to make a ballot request for yourself or for an immediate family member. Our office will send your ballot wherever you need it to be.
The deadline to request a ballot to be mailed to you is no later than 5 p.m. on the 12th day before Election Day.
To request a Vote-by-Mail ballot, you must provide:
- Full name of voter
- Date of birth
- Residence address
- Mailing address, if different from home address (ballots cannot be forwarded)
- Florida Driver License number, Florida ID number, or the last four digits of your Social Security Number
- Signature of voter (written request only)
You are allowed to request a Vote by Mail ballot for yourself or for an immediate family member. Immediate family members include: spouse, grandparent, parent, child, grandchild, sibling, spouse’s grandparent, spouse’s parent, spouse’s child, spouse’s grandchild, and spouse’s sibling.
Your Vote-by-Mail ballot will include instructions on the proper way to mark and return your ballot. Read those instructions carefully.
